Guidelines for the Preparation of Documents Requiring the Approval of the Chancery

•         Marriage Section will be serving a maximum of 6 clients in a day.

Marriage Section:

Marriage Clearances/Dispensation/Certification of Freedom to Marry


1. Couple Applicant or their representative may apply for marriage in the Parish. They must be asked to fill out a Marriage Basic Questionnaire. Note: Marriage Basic Questionnaire must be filled out by the couple applicant. Make sure that the marriage basic questionnaire must be filled out correctly and accordingly (front and back). (See Attached Form)

2. The evaluation will be done by the Parish Secretary and if found to be a chancery case, the Parish Secretary or either the Couple must send the filled-out Marriage Basic Questionnaire in PDF file at matrimony@dioceseofcubao.ph and make sure to include the couple and the parish in the email thread of communication.

3. Before sending the Filled-out Marriage Basic Questionnaire at Chancery, make sure that the desired date of marriage has been pencil booked and give them at least 15 days to get an endorsement letter from Chancery.

4. Chancery will provide them the Timeline-List of Requirements and Endorsement Letter by email.
Set of requirements will be given on a case to case basis. The Endorsement Letter will contain the confirmation of Chosen Date of Marriage, Request for Canonical Interview, Pre-Cana Seminar, and the Request for Publication of Banns.

5. The Parish will advise the Chancery and the couple for the exact date of canonical interview and
Pre-Cana Seminar.

6. The 90% of the required documents must be submitted in PDF File to CHANCERY thru email at least 3 months prior to the wedding for appreciation. Once authenticity and validity of documents have been established, the couple will be advised to forward the original documents to the parish of the place of marriage in time for the issuance of the request for Publication of Banns and as a reference of the priest-interviewer during the canonical interview.

7. Canonical Interview must be done at the Parish. 90% of the required documents are necessary for the Canonical Interview. After the canonical interview, the couple applicant will be advised to proceed to Chancery for the following reasons:

a. To submit the filled-out Application for Marriage Clearance (will be provided thru email by chancery), the result of the canonical interview and the required documents

b. Issuance of the marriage clearance/Dispensation/Certification of Freedom to Marry. Once the result of the Canonical Interview is favorable, all necessary documents has been submitted and freedom to marry has been established, the issuance of Marriage Clearance, Dispensation and Certification of Freedom to Marry can now be made. The Parish will not solemnize the marriage without the Marriage Clearance from the Chancery.

8. An appointment will be arranged by the parish secretary at the chancery. Communicate thru Ms.
Jovy Aquino at 09214386918 or matrimony@dioceseofcubao.ph. An Appointment Request Form will be provided. No Confirmed Appointment, No Appearance at Chancery.

9. No walk-in client(s) will be accommodated.

NOTE: 

For Representative and HIRED WEDDING PLANNER OR WEDDING COORDINATOR, Special Power of Attorney or duly signed Handwritten Authorization letter is needed for the process of all requirements. For relative-representative, such is not needed as long as communication is open and both are aware of the requirements and procedures. The representative can only act on behalf of the couple for the application and for the submission of documents.

Online Publication of Marriage Banns

In accordance with Canon 1067 and the Complementary Norms of the CBCP stipulates that “Publication of Marriage Banns or other appropriate means for carrying out the necessary inquiries which are to precede marriage” be made. Obviously, the purpose of these norms is to make sure that there are no impediments to the valid and licit celebration of marriage.

However, due to the global effect of pandemic COVID-19, our parishes are not celebrating community masses of which our tool to carry out the publication of marriage banns. As our usual practice, by making it known to the public, we expressly carry it out as being announced before every Mass and by posting it in the Parish Bulletin Board.

After a reflective conclusion, it is fitting and proper now to utilize social media for the Publication of Marriage Banns. It is opportune and timely to use the social media platform in bringing the church, its activities, and services to the faithful but to make sure that moral perspective and pastoral responsibility are in high regard. Attached is the suggested format for the online publication

It is suggested to publish the marriage banns on the:
a. Parish Bulletin Board
b. Parish Website / Facebook Page


Dispensation from the Marriage Bann

Dispensation from the Marriage Bann may deem necessary this time due to current circumstances. One Sunday Banns can be dispensed by the assigned Vicar Forane in every Vicariate. Two and Three Sunday Banns can only be dispensed by the  Local Ordinary of the  Diocese of Cubao thru  Chancery. Documents of the granted dispensation shall be forwarded to the Chancery every two (2) weeks.

Requirements:

One Sunday Banns

1.   Fully Accomplished Request for Dispensation from the Banns and duly signed by the parish priest.
 
2.   Photocopy of the Certification of No Record of Marriage (CENOMAR)

Note: Canonical Interview has to be made prior to the request for dispensation from the marriage banns. The interview must be in the presence of two witnesses. The witnesses must be One member of the immediate family and a friend for 5 years. Kindly use the new set of Pre-nuptial Interrogation Form and Affidavit About The Free Status provided by the Diocese of Cubao.

Requirements:

Two or Three Sunday Banns

1.   Fully Accomplished Request for Dispensation from the Banns and duly signed by the parish priest.
 
2.   Photocopy of the following documents:
a.   Baptismal and Confirmation Certificates b.   Birth Certificate
 
c.   Certification of No Record of Marriage (CENOMAR)
 
d.   Marriage License or Civil Marriage Certificate, if civilly married e.   Result of the Canonical Interview 
 
Note: Canonical Interview has to be made prior to the request for dispensation from the marriage banns. The interview must be in the presence of two witnesses. The witnesses must be One member of the immediate family and a friend for 5 years. Note: This only applies to a couple(s) who are going to be married within the territorial boundary of the Diocese of Cubao. Likewise, for couple(s) who are going to be married outside the Diocese of Cubao, and happens to have lacking Sunday(s)  for publication of marriage banns,  they must be advised to get assistance from the Archdiocese/Diocese of the place of marriage. 

Registry Section

Partial Change/Full Reconstruction of Documents

Every transaction will be thru emails that have to be initiated by the Parish Offices Staff.
1.   An initial assessment of the request will be made at the parish.
 
2.   The Parish will ask the client(s) to send to them thru email the necessary documents to process the issuance of the Request and Affidavit for Partial Change/Full Reconstruction.
 
3.   Once the request has been established, the parish will communicate/arrange at chancery for the schedule of appointment with client(s).  Kindly email the  Request and  Affidavit for Partial Change/Full Reconstruction thru  registry@dioceseofcubao.ph & chancery@dioceseofcubao.ph and include the client(s) in the email thread of communication. To know the status of the request kindly reply to the email thread of the same subject and/or may call 09667854545 (Mr. Dondon Fabroa) and/or 09055228538 (Mr. Gil Bautista).

4.   The parish will advise the client(s) to bring the original copy of the documents asked during the assessment of their request for validation once their appointment is confirmed by the Chancery. An Appointment Request Form will be provided. No Confirmed Appointment, No Appearance at Chancery.

5.   Transaction in every case must be dealt with in a maximum of 14 days (2 weeks).
 
6.   No walk-in client(s) will be accommodated.

Procedure to acquire Certificates of Baptism, Confirmation and Marriage kept at Chancery Archive.

1. Requests must be processed at the Parish and must be sent thru email at Chancery.

2.   Once the request has been established, the parish will communicate/arrange at chancery for the schedule of appointment with client(s). Kindly email the Request thru registry@dioceseofcubao.ph & chancery@dioceseofcubao.ph  and include the client(s) in the email thread of communication. To know the status of the request kindly reply to the email thread of the same subject. An Appointment Request Form will be provided. No Confirmed Appointment, No Appearance at Chancery. 


3.   Transaction in every case must be dealt with in a maximum of 14 days (2 weeks).
 
4.   No walk-in client(s) will be accommodated.

Remarks:

For Representative or AUTHORIZED REPRESENTATIVE, Special Power of Attorney or duly signed Handwritten  Authorization letter is needed. The representative can only act on behalf of the client and for the submission of documents. The Client must be reachable for inquiry at all times.
